This is a bit literal but to kick things off, perhaps:

A Íácób, is tusa mo chroí agus m'anam, gach anáil a tharraingím agus gach brionglóid a fheicim
"Jacob, you are my heart and my soul, every breath that I draw and every dream that I see."

(I don't think Irish likes the combination mo gach* )
