For me "Places of my heart" means places where my heart rests, where I feel home, where I feel comfortable in highest level...
Places where I forget about my sorrows and sometimes about myself; places I share with people who remind me of what counts for real and what kind of person I would love to be...
well, yes,... something like that - would appreciate a poetic translation...

This is a bit tough. The Irish version Red suggested is absolutely fine, and means exactly what you're looking for. All the meaning you want associated with it is obviously a very subjective interpretation.

Word for word, Red's suggestion is fine in any case, if that's OK for you, then go with it in my view.
