I think it is: Go deo 's gan choinníoll, mo chroí

If, as I assume, you'd be addressing her/him (rather than speaking about her/him), you'd need to make the change indicated here (vocative case):
Go deo 's gan choinníoll, a chroí

Otherwise, it looks fine to me.

Dont just take my word for this but i believe " Go deo 's gan choinníoll, mo chroí " translates to "Forever ' s unconditional , my heart" but my irish is more then a bit rusty. I'm not sure if this helps at all. if not then ignore this. ;-)

"Mo chroí" is literally "my heart," but used as an endearment, "croí" means "love/beloved." Also, as it's direct address, so you need the vocative case: "a chroí."

In case it is not an address but short for: You are my heart forever [...]
then no comma, and "mo chroí"
